As a patient advocate for our clients, we feel that we have many responsibilities, some of which are to help you and your family understand the complexities of care. Our personal case manager will spend time with you and your family to address the unique needs of each patient to help determine what is needed today, as well as to determine what may be needed in the future. Our personal case manager will provide assistance in navigating the complexities of care to determine what is going to be best for you.
Your personal case manager will be involved with the planning for your financial options, help with understanding care options, providing information and access to complimentary services (including home safety remodeling, providing recommendations for financial and legal services), assessing the current needs of the client and family, anticipation of the future needs of the client and family, assistance in the coordination of home based care (including medical, guardianship, care management, and non-medical), as well as making sure the highest standard of care is being provided.
